School Supply List
Required Paperwork
- VHCS Application (if not already completed)
- Copy of Student's Birth Certificate
- Records from previous school or homeschool (if applicable) OR records request for us to get the records
- Medical Forms (please fill out completely)
- Immunization Records/Exemptions
- School Physical Information (from physician's office)
- Photo/Video Release Form (please provide a photo copy of each individual's driver's license or photo ID)
- Authorization for Student Pick Up
- Any legal paperwork around custody arrangements, name changes of any kind, etc.
